Disclosure of payment: Payments are due in full at the time of services rendered.  DUI Classes are $60 per 2 hour session.  The number of sessions is usually determined by the referral source, courts, or assessor.  Assessments are $150.  These services are not covered by insurance.

Required documentation: Once an assessment has been scheduled it is the client’s responsibility to bring all documentation regarding the incident to include, but not limited to: police reports, date and details of the incident, type of intoxicant, results of any field sobriety, blood, and breathalyzer tests conducted, and any applicable history of prior DUI/DWI offenses and/or subsequent treatment.
